Flu Shots will again be offered at each of the buildings in September and October 2020. You are invited to have the flu shot administered at your building. If you are not able to attend at your building, you are welcome to have it administered at another building.
- If you are ON district health insurance, the shot is free. This includes your spouse and children (6 months and above) if they are on district health insurance as well. Please remember to attach a photocopy of your insurance card with a completed consent form. The photocopy helps the staff administering the shots confirm that you are on district insurance.
- If you are NOT on district health insurance, you need to complete the entire consent form, and attach a photocopy of your insurance card and payment ($32.00) to the consent form. Please note: Hy-Vee will bill any insurance; however, they cannot guarantee there will be no co-pay for those not on district health insurance.
